Government has been busily engaged in jacking up the standards --- and costs, of health care for many decades. You reap what you sow.
These days health plans usually aren't permitted to exclude procedures like joint replacement, organ transplants and such. If you want access to very complicated and expensive care, expect to pay for it.
And of course the promise of Obamacare is to provide access to this kind of care for people who can't begin to pay for it. Expect to pay a lot more when someone is looking around for someone to pay the bills.
