There was no previous record anywhere of this doctor providing a service, and then out of nowhere a bill arrived in the mail. All we wanted was proof and they were unable/unwilling to provide that. >>

I had good fun turning the tables on a doctor one time.

I got a call from a woman asking for service on her furnace. I made arrangements to come right out to make repairs, and she agreed to my fee of "$85
