One thing that jumped out at me...
...from where the TV will be mounted on the wall above the fireplace...
Unless things have changed a lot over the past few years, this is a serious no-no. It's convenient, it looks good, and it is HORRIBLE for the TV. Also, that's usually regarded as a bit too high for a TV. The bottom of the TV should be 3-4' off the ground (roughly eye-height while you're seated).
If you can change that somehow, I would.
If you have access to the wall cavities from the attic, it shouldn't be a big deal.
You might try a different plate for the HDMI. You wouldn't have to worry about connecting to the keystone plate if you used this:
I have one, and it's fine.
