How many non-rich people can afford to do this, after paying higher tax rates to fund the basic national system?

In France, almost everyone.

My wife's parents are clearly working-class. They are in their mid 60's. The total annual cost for their private insurance is about $2500 for both of them. (The public insurance pays the public reimbursement rate, the private insurance pays for whatever additional the private hospital charges above the "public" reimbursement.)
