2) Create a system where patients care about cost - and, in that large majority of situations where it's feasible, have access to the necessary information so that they can include cost in their choices.

I think one of the biggest problems with most government intervention -- not just in health care -- is that it insulates people from having to bear the full cost of their actions.

A great example is deposit insurance. It insulates depositors from the full cost of a bank failure. The result is that depositors do pretty much no due diligence to ensure that a bank is financially sound (and banks have no incentive to compete for customers based on financial soundness).

If you forced people to bear the full cost of their actions, we wouldn't have these problems.
