the income-based Medicare surcharge, which I view as an insurance premium, not a tax.

The congress (but perhaps not the IRS) view it as a tax, though. Because if it were an insurance premium, it would mean that social security (and medicare) are insurance, not welfare, and they could not change the amount of the policy ex post facto, as they seem all to willing to do.
