it's almost a racket when you think about it.

it they get a couple big corporate customers, they can charge almost anything -- the Corp just deducts the premiums as expense & passes some of the cost to customers.

they only have to avoid being outrageously more expensive than competitors and they have to be nice ( reject the CEO's daughter's claim and lost the account )

for individuals, they can charge literally anything and don't have to be nice at all.
