counting softly while he lies quiet/still/with eyes closed. I start at zero and count slowly up to 100 (though they have both asked for random end numbers like 77) and then count backwards from there back to zero. I count pretty slowly and it almost always works for us.

I will second this idea.
I think it is hearing your voice, so they know you are THERE.

I did this at some point with both the kids - not counting, we called it having a scene.
They had to be quiet- eyes closed, and i would tell them verrrrry sloowly and calmly and quietly...
we are walking down a path <5 second pause>
the sun is shining, but it is (cold/warm/windy) <5 second pause>
there is a stone in the path <5 second pause>
you kick it and watch it fall through the moss <5 second pause>

just whatever kind of scene you want to put in his mind.
even now the elf asks me to *talk him to sleep* as if my voice is equivalent to white noise
