In the UK and Canada, if the NHS denies you, you can't get the operation there no matter what. Doctors are prohibited and so are hospitals. If they say no, you won't get it in the country. The system won't allow it.

For Canada, you can come to the USA if you got the cash - when you are denied, or put on a never ending 2 year waiting list that never gets shorter.

For England, you are usually screwed unless you got money to fly to India for care there...or maybe Thailand. And money for the operation to save your life.
