For the 401k I would roll to a Vanguard IRA account and use the brokerage services for their no fee ETF'S:

1/3 in BIV - Intermediate-Term Bond (Investmnt Grade Government bonds for the most part)
1/3 in VTI - Total Stock Market (US)
1/3 in VT - Total World Stock

You could even do the cash the same way in a Vanguard Taxable account.

In addition to the no fee ETF's, Vanguard's expense ratios are among the lowest, if not the lowest in the industry.
