Look at the back of the W-4. There's a mess of stuff on there to deal with dual-earner couples. Even that's not perfect, though--it can't cope with variable incomes.

Note that at various times I've been given a single-sided photocopy of a W-4 instead of the real thing. If you're form is one-sided, get a real one.
