I realize you're violently opposed to debt, but the need for a credit card in his situation is legitimate. Without one, he'll be unable to travel for externships and residency interviews - how will he buy a plane ticket or rent a car? And while I'd be happy to buy him a book or instrument that has to be mail-ordered with a credit card, and let him pay me back, I'm sure he'd consider it an embarassment.
Going on an all cash diet would be great, it's just not practical.
(He's also not nearly in as much debt as some of our classmates - he'll easily pay his back by his 2nd or 3rd year of practice, unless he turns stupid and locks himself into a big mortgage and a leased Lexus.)

Why does he need another credit card? He's already got plenty of debt thanks to the medical student loans. A credit card would just make it easier to get deeper in the hole. I think that Capital One may be doing him a favor by denying him a credit card. You might get him to see if he can live on a cash diet because it will keep him from mortgaging even more of his future on debt.
