I tendered some shares of Pfizer for shares in their Zoetis subsidiary back in September in what was supposed to be a tax-free exchange.

I just downloaded my Vanguard tax information into Turbo Tax and they are reporting a capital gain for the transaction.

Is there some IRS form I need to file to show that this is a tax-free exchange, or has Vanguard make an error in reporting?

The information you reference later in the thread confirms that this was a tax-free exchange. Before we can tell you how to report this on your return we need to know how it was shown on the 1099-B, specifically, whether holding period and basis were reported to the IRS or not. Remember to look at only the 1099-B, not the supplemental information, in making this determination.
