I think they did a great job on TBC except with their instance planning. There are far to many "easy" instances and the progression on the hard stuff is a bit too steep.

For example, at level 70 there are 16 new 5 person instances in TBC on normal mode. All of those can be done on heroic, which gives 32 possible destinations for a party. Those who levelled up quickly had plenty of instances to do with guildmates early on when every place had upgrades. Those who levelled more slowly or started over as a shaman/paladin can have a very hard time getting a group for the lower level places. Also, everyone needs a different place, so it can be hard to find 5 people for a specific instance outside of PUGs. Of course, even with all these places to go, the heroics really don't upgrade blue gear from the other level 70 instances much, making them only minimally useful.

That leaves us with the raid zones. As mentioned in the Warcraft forum, the trash respawn timers in TBC make it very hard to learn encounters for casual guilds. We started Karazhan with 1 day a week. Since then we have ramped up to 2 raids with 2 days each. We just killed the Prince, Nightbane and Ilhoof for the first time last month. In the process we have lost half of our best players who felt they were moving too slowly. We're working on the High King now and will probably get him in September. Maybe October for Gruul at our pace. Chances are we won't see the Eye, Hyjal, Black Temple or even Magtheridon before the next expansion.

Some might say that we should work on PvP for gear upgrades if we can't make it to the top instances. However, the real loss is the content. We were halfway through BWL and AQ20 when a release date was put on TBC. After that we lost all of that progress as interest in the old world content dropped off. We never got to finish BWL, AQ40 or Naxx. Even now we can't get a few people together for the old content because people are too busy with the current stuff. So, the cycle will continue for the next expansion as well.
