I think we all recognize that providing insurance for preexisting conditions and removing lifetime caps will cover some with expensive medical needs and that we premium payers will pay for it.

But the hope is that by forcing young healthy people to pay into the insurance pool the increased costs will be covered and the premium increases will be modest.

Of course the poor cannot afford insurance and need subsidies.

Obamacare is a great experiment. We will have to see how it comes out, and perhaps adjustments will be needed. But at least America finds basic health insurance for some who needed it.

Yes, co-pays and coverage gaps (like the donut hole in Medicare Pt D) encourage people to be more careful in their health spending, which given human nature is good for those paying the bills. But then why was plugging the donut hole in Pt D such a big deal? Did anyone seriously object?
