I would suppose that among the decisions an engineer must make is how much quality to design into a project. Too much or too little is a poor decision, I would suppose.

Quality/Reliability has a cost. The business case for a product estimates the price range target customers are willing to pay. Over estimating the quality requirements means that the product probably never will be built. Under estimate the quality requirements and the product will fail.
