If you followed the details of the Challenger failure, you might recall, some of the engineering staff knew the o rings had an issue at low temperatures. Looked at one way, the failure to admit the mistake prevented scrubbing the launch and "fixing it". But I know what you mean. Some mistakes, admitted to and fixed or not, are just to big to survive. For others, surviving the mistakes is how progress gets made. I admit to being troubled by the "overheating" root cause...that looks on the surface to be risky engineering practice, if true. On the other hand, I had a friend "overheat" my 240Z after the water pump failed and bit into the radiator, while he drove another 10 miles.... Never could get the head gasket to seal even after shaving the head and replacing it. No fire.
