Let me try to explain it better.

My 401k account is managed by Fidelity. I can view transaction histories online. Every other month there is a line for "Remove Shares" against my company employee stock fund. The total of these "Remove Shares" for the current year is $5.36, which I now assume to be the account management fee that Fidelity charges.

However, there are no lines with deductions against any of the rest of my mutual funds. I was expecting fees to be listed like I described above, which had previously led me to believe that no (other) fees were being charged.

Does anyone go out and check their accounts to see what fees are being charged (how?) or does everyone just assume that whatever the prospectus says is correct?
