I did exactly what you are proposing with my 401(k) and Ameritrade was most accomidating.

I think I sent an Ameritrade account application in (without the $2000 they say they need.) I told them I needed an account #, since the rollover IRA would be funded from a 401(k). They set up the account and provided me the number. I then sent my paperwork into my employeer with that info.

They cut a check to Ameritrade for me and then sent it to me. I just forwarded it to Ameritrade and WALA!
