1. Spin a tee up in the air and whoever it points at tees off first.
2. Depends on the course I would suppose... I've never played with a caddy. At the course where I usually play, we generally tip the staff a couple of bucks to clean the clubs at the end of the day.

5. Depends upon the agreed upon hole or 9 that you bet the drinks on...
6. Mulligan? Isn't that called 3? Honestly when playing w/ my dad, you get 1 on the first hole and that's it... Otherwise we're sticklers to the rules. With my golfing buddy, there are no Mulli's, but he's about a 4.5 handicap
