My wife and I have an individual 401K that covers both of us (as you may know, an "individual" 401K can cover two people when they are spouses). Ours is a Costco offering that is administered by SafeCo and American Express. SafeCo handles the actual investing component while AmEx provides all the paperwork/tax services. We pay a flat annual fee of $150 for our plan. We've been very happy with it so far. They have an excellent online portal for viewing/changing your account and offer a good selection of fund choices. Outside of their selected fund offerings, they also allow for investment in virtually anything via ShareBuilder.

Although I don't know if Costco is still advertising the service, I know that SafeCo/AmEx are still accepting new customers directly.
