One little thing to add to Phil's excellent post.

Filing form 8919 is essentially a request to the IRS to take a good hard look at the employer. Unless there are multiple people in his situation, the hospital will probably find it trivial to figure out who the whistleblower is. How that will affect the employment situation is a big unknown.
