I have a 401k that I need to rollover soon. I have a good number of years until retirement so I am okay with fairly aggressive investments.

My questions:

1)What is a good, yet low in fees, investment company to use for this rollover? --I have over 5k in the account. Vangaurd, Fidelity, ShareBuilder, etc..?

2)What are good investments (funds) that I should invest in? Think I should be looking for "no-load and low-load" funds to avoid more fees.
One calculator suggested a mix of:
50% Large Cap stocks;
20% Mid & Small cap;
15% International;
10% investment grade bonds; 5% high yeild (or 15% Municipal bonds instead).

Know that I need to have the funds moved w/o being put in my name to avoid taxes.

Any suggestions on the above would be helpful.
