Does this ring true? Seems like I should be able to find out exactly how much I paid in fees over a given period. The rep. was not comfortable saying that I could generalize .04% (or whatever) from the entire fund to .04% to my balance. Can anyone explain to me how that could be different?

Twice I have referred you to the Department of Labor papers on 401k fees. Twice I have suggested you read them. If you do so, much of what you're asking is explained there.
