No insurance company is going to be issuing private policies, or through an exchange unless the goverment picks up all the losses (and they will be massive) since the government will 'set the rates' which are guaranteed to lose money by the tens of billions.
Yep, the 'insurance companies' will make money, only a few percent, administering the plans and the tax payers will be on the hook for a hundred billion a year loss after another 100 billion subsidy program to give it away at bargain discount prices (free and highly subsidized).
You are quickly going to find that private policies will be a thing of the past for everyone. You'll wind up on government Plan A or B or C or D.
Some of us would like to see healthcare 100% administered by the governments. I say get rid of private health insurance companies altogether! But that's set for another time. I'll enjoy this victory for now.
