The law, however, does not apply to the surcharge for those above a certain income level. Their base stays the same but the surcharge can increase.

Exactly, and they really increased the surcharge! Most people pay in the neighborhood of $98 per month for medicare. I got socked with around $220 a month. Holy Moly! But, in 2009 I did not take any RMD disbursements, so my 2011 medicare fee should go down for just that year. Crazy.
