The only part of this I can address is the hold amount. Rental companies pre-authorize some amount, which when the rental itself is already paid for is usually not large. It is possible for a pre-authorization to expire before you return the car, but at least they knew that, at the time, the card was valid.

I recently rented a car in another country, not prepaid, and received a notice from the credit-card issuer that the agency had pre-authorized 10,000 "unspecified units of currency" and, while they had approved the authorization, the card was now maxed out. I don't expect an overlimit fee, because the actual charge will be less than $1,400, which is the value of 10,000 of the country's currency. At this point, the authorization has expired, but the actual charge has not posted.
