This has nothing to do with words. Rather, it has to do with a certain doctor, who, upon walking into the patient's hospital room, asked the attending nurse: "How is the patient doing?"

"He's in a lot of pain."

The doctor pulled the nurse aside, and instructed her that the proper thing to say is, "The patient thinks he's in pain."

The next morning he met up with the nurse in another room. Just to see if she'd learned the lesson the day before, the doctor asked: "How is the patient doing?"

To which the nurse replied, "The patient thinks he's dead."
