Was "4" ("Was referred to as Dr. Pimple Popper, M.D.") before or after she became HIV positive trying to save folks in Africa, returning to the US to try to reconcile with her former soldier mother and obsessive but well meaning snooty brother, only to fall in love with the new resident widower brain surgeon with two kids of his own who despite his intelligence and empathies couldn't really deal with the HIV stuff and neither could the small town, and she felt compelled to banish herself back to Africa nearly to never be heard from or of again?

Either that, or because she played a dermatologist on Seinfeld.
