What is the death benefit? Is this a supplimental policy to a policy you have outside of your work? Is this your only policy that your family will depend on?
Do you need this policy?

What happens to the policy if you leave your employer? Does it cease or can you continue to pay the premiums yourself? How long do you plan to stay with this employer?

The reason I ask these questions is that you may not want to be paying the premiums on a whole life policy if you leave your employer unless you have health problems that eliminate new policies due to cost.
