When I see someone parking in a handicapped space, including those spaces for opening van doors, I call the Sheriff's Dept. If there is someone in the car, I warn them first.

Just to be clear, I was complaining about cars with handicapped plates parking in the "van accessible" handicapped parking spots. But I think the laws and enforcement vary from state to state.

I think it's standard to require at least one "van accessible" handicapped spot for every seven normal handicapped spots.
