I understand that I need to list all my stock, option transactions and I plan to begin to convert them from my own spreadsheet to the proper format.


1. Where can I find such a form for 1999 from the web?
2. If I can't find it, can I use spreadsheet and create one?
3. In what order shall I list all my trades? In the order of Date acquired, or in alphabetic order of the security symbol as primary and Date acquired as secondary (this is to see if wash sale rule applies) ?
