Can you make a withdrawal from a 401K say at 60, and then not make another until 64 and then again at 66 or does the automatic withdrawal calculation kick in as soon as you make the first withdrawal?

Above age 55 you can do whatever you want, until age 70 1/2, when you face mandatory distributions. You can get more information in IRS Publication 575. NOTE: this is tax law. Your plan is allowed to have and may have additional rules.
