Does a name like McMurphy precede a name like Maker in an alphabetical list?  I know I used to see that sort of thing all the time.  But nowadays, I often see a name like Maker coming first.  Is there some clear rule or guideline about these cases?

There used to be a special case for things like this, and yes they would go first.

However, nowadays virtually all sorting is done by computer, and reliably spotting all the special cases is something computers don't do (yet?).So it's typically a strict alphabetic sort of the letters, possibly case-sensitive (all capitals coming before all lower-case) but probably not. Which I find easier to work with anyway.
