My dentist is 30 miles away. If I make a dentist appointment I try and also do all my other 'big city' stuff at the same time, but since my main reason for going is the dentist the mileage to the dentist and back is deductable...not the 'other stuff' mileage tho' The primary purpose of the trip was the dentist.

i don't think it makes sense otherwise.

my Dentist is about 15.1 miles away ..far enough, that i stop for dinner at a favorite pizza place every time.

i don't deduct the extra 1.2 miles ..but do deduct the 30.2 *

* actually ..that's the scenic route ..and i only deduct the most direct route.
