Are you--and/or your club--a member of NAIC (National Association of Investors Corp.)? I'm almost certain you can find samples of the forms you need through NAIC's website at www.better-investing.org, although you may have to be a member to access them. I'm also the treasurer of an investment club, and I can't imagine doing my job without the help of NAIC's software. Not only does it do all of the accounting, with very little work on my part, but there is also a tax form printer issued annually that prints all the forms for me. I'd highly recommend their software products--and I don't think you have to be a member to buy them.
