Good morning.

I was reviewing my tax filings earlier this week, looking forward to filing and being through with the process, and noted I'd not seen any information on the 529 & coverdells I opened for my children this year.

I contacted the 529 administrator, and found that since I was not a resident of the state plan I was contributing to and since I did not recieve a distribution from the plan, no documents were going to be mailed. Exactly as I expected.

I contacted customer service at the broker who holds the children's coverdells and asked if I ought to anticipate recieving any documents I'd need for my tax filing. The cs gent asked about activity in the account. Not that I thought it relevant, but I told him I had 1 pucrhase in one coverdell, and one purchase and one sale in the other. No distributions, & all contributions were for TY2003.

He then said I'd be should recieve a 1099-R.

After looking at the 2003 1099-R instructions, I think I should be getting a form 5498-ESA reporting the coverdell contributions. I've looked on the forms & pubs section of the IRS' site, and can't locate the 2003 5498-ESA to save my life. The 2004 ESA version is sitting out there, pretty as can be, but no 2003, and I can see the 2003 IRA & MSA versions, just no 2003 ESA. Is this one of those instances where if it was a snake waiting to bite me I'd be dead?

Anyone who can verify that 5498-ESA is what I should be getting, and where I can locate the 2003 instructions & form?
