Do residential builders just not give a crap? Does the first guy say, "Hey, that attic access door/ladder opens right into a wall," and then the second guy says, "Oh crap, you're right, but screw it it ain't our problem?" Is that how they think? And does nobody oversee them? Does the general or his PM not walk the job and check on the framers' work?

Kind of, yes, especially when it's a development with lots of houses that are being slapped up assembly-line fashion. You've heard the expression "Done is better than perfect"?
In residential, it's often, "Just get it done, period."
If they get five things wrong, and the buyer complains about one, they'll fix that one, and the other four things get chalked up to the "time saved" column. So for the one thing they fix, it would've been more efficient to have done it right in the first place, but overall they come out ahead.

As far as the GC or PM checking, they have a checklist: Studs @16" oc? check. Bath fans vented to outside? check. Water comes out of faucets? check. etc
As far as design flaws (attic access placement) or QA (water coming out of the correct faucets), they just don't get bogged down in those details.
