I used AXA Financial and they charged a $400 annual fee plus fees on transactions. I switched to Fidelity for that reason. Fidelity does not handle loans. So if you think you might take a loan on your 401k, don't go to them.

I believe Vanguard was looking into offering individual 401k but hadn't done so at the time I made the switch.
