One example would be where a couple is fatally injured in an accident. In some cases who dies first, affects who inherits.

Ditto trusts where rights change with age of individuals.

I suppose the case that most needs an update is when an ex-spouse is listed as decider. Ditto step children. As to aging or deceased deciders, common practice is to specify alternates if the person listed is unable to serve. Given enough time, most such lists probably need an update.
