It sounds like you ran into a limit of coverage in a 12 month period, fwiw. (Really just guessing, but I have seen such a thing for my dental coverage. One implant last year was covered. The clock started over the next calendar year and covered the second one.)


That's common for dental plans, but shouldn't happen under Obamacare where you are covered for "medically necessary care". If the doctor says both knees need new joints, they can't limit you to one per year.

I wonder if there are vision plans that limit you to a monocule if you're vision is only bad in one eye? It would be a very Republican solution.
