If you haven't already, then you should consider having a sleep study done to find out if you have a sleeping disorder.

If you are in the process of becoming a doctor then you might consider going with an HMO type of organization. I have several friends that are HMO doctors and other than having to occasionally be on call it is basically an 8 to 5 job, which is why they like working for an HMO. They also don't have to deal with the business side of it much.
