If you force new minimum requirements into an existing policy then it really isn't the same policy is it.

Nobody ever said the insurance companies would offer the same policies as before the ACA.

The point is simple: The stuff that was not "real insurance" was essentially removed--but the cost of what was dropped was very small. Which is why the insurance companies included it in their policies with a highly-inflated price. It is what made them so profitable.

When the ACA demanded actual medical costs paid to be 80%-85% of premiums collected, the insurance companies were required to refund premiums they had overcharged.

Premiums would go up due to including more real medical costs being covered by insurance, which is reasonable. However, not everyone uses every medical care/service covered in an insurance policy. So, ALL insurance policies are, in one sense, a ripoff--because of that fact. The same statement can be made about many other products consumed by the public but not used by all. The fire department, for example. There are lots of other examples, but you get the idea. There are lots of things funded that you don't need, but others do. The cost is spread over the public because everyone benefits--directly or indirectly. That applies to health insurance as well as many other insurance coverage--think flood insurance. Funded by the US public because there would not be any flood insurance otherwise.
