There are people on the Taking Care of Parents Board that have ton's of experience with this, I would suggest posting there, too. With my mom, she had her will, medical and durable POA as well as medical directives done all at once. But I'm her only living heir and she has few assets so it was fairly straight forward. I would think a trip to the original attorney (who drew up the latest will) with your dad, and siblings if possible, would be in order to make sure everything is spelled out as he wishes.
