Walmart/Sam's Club sells drugs to people without insurance at the market rate for non-insured. However, they bill the insurance companies the insurance rate. The insurance companies turn around and insist that their members be charged the copay rate according to the insurance policy.

So . . . a few months ago, when I was still employed, I had insurance that called for a $15 copay on generic drugs. If I used the insurance at Sam's Club Pharmacy, they would charge me $11 and change because the price they charge insurance is less than the $15 copay. But if they don't run it through the insurance, they charge me $4 because it's on the cheap drug list.

The pharmacy associates were quite open about this situation and helpful in getting me the best price.

Now that I'm retired, I'll have to ask them about how that interacts with my HDHP. I suspect I'll have to not let it run through insurance (and thus, not count toward the deductible) in order to get the cheap price.
