While we (Canucks) have universal healthcare for the basics we also have private insurance companies that cover pharma, private rooms in hospital, physio, glasses, eye exams, travel insurance and a host of other stuff,

We have had Medicare supplemental policies available for decades that cover extras like that. But the largest cash flow is for the medical costs that Medicare does cover. The insurance companies wanted to skim that larger cash flow, so we got "Medicare Advantage", that funnels all the money from Medicare through the hands of the insurance companies so they can skim a profit off that, as well as bundling secondary services with the primary medical.
