You would need to move assets into the trust or retitle assets.

That needs to be done when the trust is established. Without that step, the trust isn't accomplishing anything.

Once put into the trust, assets don't need to be retitled because of a change in trustees. The trust itself continues unchanged. There's simply a different manager in charge. Much like changing the president of a corporation.
